I personally recommend the standalone version rather than the combined version. The standalone version is placed in the center of the back, while the combined version feels a bit crowded.
If the mod isn't working, please check the load order to see if it's being overridden by other body tattoos.
(However, unless you know how to merge tattoos yourself, you can usually only install one tattoo for most body areas.)
