There is vanilla audio bug with extendable ladder which is causing loud sound appearing out of nowhere.
It's played always when the ladder entity is loaded and it's already extended. The reason of this is that the Sound SFX or exactly its Actor Mixer is missing attenuation and 3D Spatialization.
Here is how it looks (sounds):
Another issue is security turret deactivating sound which has same problem - it can be heard from very big distance. This sound has defined attenuation and 3D Spatialization, but attenuation is very weak and up to 50 meters from emitter and it doesn't mute the sound fully. This fix changes the attenuation to max 20 meters with muting to zero.
Installation
First download this mod and unzip to game folder
Run the game
Usage To activate the mod you don't need to do any actions, it will be enabled automatically
Uninstall Save and exit the game and delete files "SoundSFXFixes" from "archive\pc\mod" folder
