Sorry guys I haven't even touched this game lately but here's a few answers for most of the questions.
1): You only need 1 file to make it work, meaning you use the tweaked version and that's all you need, or you use the lore friendly, that's all you need.
2): Mods is universal, affects everyone, works outside of the characters creator and is compatible with the BEMP.
3): No it is not compatible with other eye mods.
4): As far I know the mod is still working. For some reason the nVidia plugin messes with the eye textures for some reason, so the original works for certain people, tweaked version was compiled with GIMP so that's the work around. If the mod breaks I'll fix it, and with the release of the new mod tool by eham I'll update if need be. Sorry for the late replies guys!
Loved this mod for many years, which is why I'm sad to report that this mod breaks when used with Frosty Tools. If you're freezing on the initial load screen, and you're using Frosty as your mod manager, this mod is why.
It still works like a charm when run with DAI Mod Manager! But if you attempt to let Frosty import it (that is, let Frosty try to auto-convert it to an fbmod), it'll cause a freeze on the initial load screen.
The workaround that I ended up using personally was to manually convert this mod into a Frosty mod, which then worked fine.
There are a few tutorials out there on how to convert daimods to frosty mods, but a couple weeks back Zachillios generously gave me permission to upload my port to Nexus! You can find it here: https://www.nexusmods.com/dragonageinquisition/mods/3908
Zachillios's original daimod version still works great when used with the DAI Mod Manager though!
Sadly this mod freezed my game before the journey to skyhold cutscene, after I uninstalled this mod I could pass the white screen (waited like 10 seconds)
This mod slowed my loading down insanely, war council went form instant load to 18 seconds. Every loading screen got 15-20 seconds long, even in my inventory takes that long for characters to pop up. And the tweaked version game opens to loading screen and stays there forever.
heads up to anyone who uses this mod and is googling why they're getting a white screen on the cutscene to skyhold: it's this one!!! This is the mod!!!! It's lovely but immediately upon removing it, the problem was solved and load times improved exponentially. Luckily the optional files - not the main ones!! - don't seem to cause the problem.

In my endeavors to figure out which mod was giving me the white screen, I also tried just taking them all out, but that included the eyebrow mod I used and frankly? I can't stand seeing my inquisitor with hellbrows for the entire cutscene, so I went through and tried taking out every single mod individually until I figured out it's this one T_T I'm just happy I saved even one person the trouble lol orz
Another note to people scrolling though here in the future, I just wanted to add after receiving a question about it that I use the "HD Eye Textures - Lore Friendly Tweaked" option from the update files without the main file. The question was whether you need both for the mod to work, in my experience you do not, just using the update file is fine.
in my case, neither works. With the optional file, it does not even load the main menu, with the main file it loads the main menu, but cant get to character creation :/ (used frosty auto-conversion)
Is there any way to alter the whites of the eyes to be pure-black for the Qunari in the game (or at the very least replace them in the character creation menu for the Inquisitor)?
I loved the way that Qunari eyes looked in Dragon Age 2, but unfortunately I haven't found a mod where anyone has even attempted it. I am more than willing to create the mod myself, I just need to know if it is possible and a nudge in the right direction as to how to begin trying.

the alternative one does not work for me, but the otherr one seems to load - another mod was causing the issue. Weird
