Load game, walk a few steps, listen to the sing-song and it should pop.

Stick in your saved game folder.
If you can't find it on load screen rename the bit before .DAS to a save file you dont mind deleting.
If you can't load it because it is a save made on a "newer version of game" go to your DAI directory > Update > Patch > Open package.mft in notepad and change version number. Mine was version 12.
Supposedly the achievement doesn't pop sometimes when you get to Skyhold and you have to fast-travel somewhere but mine popped without needing to travel.
