The standard grenade launcher, produced by Stent, is a semi auto GL housing 3 40mm grenades before reloading via break action. The grenades do a base 85 damage for a total weapon weight of 4 pounds, and have an explosion equal to a landmine. the grenade launcher being inspired by a mix of Milkor's Striker and MGL designs.
The grenade launcher will not start spawning until level 15 on Brotherhood of Steel, and level 20 in vendors & on raiders.
Scoped Grenade Launcher
Same as the standard grenade launcher however instead sporting an ACOG scope to make aiming easier.

Handcannon
The unique variant, it is a stripped down version housing 2 40mm grenades that is one handed, grenades now doing a staggering 105 damage

DC Rearmed
as for what DCR means, DC Rearmed is a series of mods, similar to Dak and co's Commonwealth Weaponry, to add weapons to 3 that fill roles that weren't otherwise in a series of mods instead of in one big mod like AM, WEP, or ZLA, in this case the role of both a later game small guns weapon as well as a LMG to more nicely fill the gap between assault rifle and minigun
