FWIW after the years. Had the same issue, a perk with no name in my skills list and radiation immunity. Hell I could even drink the water in the mass fusion reactor room. Seems that I got this perk from the mod Fusion City Rising, probably when stepping on the vault door at this decrepit factory or so. Fo4edit says that Fusion City Rising grants this as temporary radiaton immunity. Anyways, the Perk ID is 000a2775 and you can remove this in the console with player.removeperk 000a2775.
do you have the companion mod *Mistress Passion* by any chance (or, desire, exotic, and what ever other names the author used, same companion mod different looks and names)
if so her max affinity perk gives you immunity to radiation.
in that case it looks like you get it from the children of atom, in the glowing sea. (speculating here, i have never actually got that far in the storyline before xD)
thier is a immunetoradiation perk in the game, but this is primarily used by enemies, however their is a quest reference to it, which i would assume you would get it via this quest, however i would imagine it to be temporary. as i doubt you would get this permanently as it is basically cheating.
So the funny thing is that I also have never been to the glowing sea. I got caught up so much in modding and settlement building that I kinda stopped the storyline.
Just curious, but when I do a help search in the console commands, I also come across this "DFOB: PerkImmuneRadiation_DO 001b19bb". Do you know if that's part of the base game? Wondering if that's something I can try to research more.
looks like a mod either added it or modified it (or it might even be a dlc file that has modified it, especially far harbour as that also has children of atom related content), and this mod would probably be close to the master files, so check the mods closest to the top of load order. but either way given the form id that you posted, means it is high up in the load order (very close to or literally the master files themselves, ESM)

so if it is referenced again with a different id, that means a mod or dlc has either modified it or added a new record of it, as the form id represents where the record is located as such higher numbers and letters = the mod is further down in the load order.
the absolute best way and quickest way of determining if a mod either added it or modified it, is to load all mods in F04Edit, and 1 by 1, click on the Plus sign next to each mod, look for a Perk Record, if the mod does not have it, check another mod, keep doing it until you find a perk record, when you do, click on the plus sign next to the perk record, and locate the perk in question "immunetoradiation* then if you find it, you will now know what mod added/modified it.

from the second example, we now know that the mod *TF - Perks Plus - Overkill*, has a Perk Record (This record is added because this mod edits perks) and it modifies rank 1 of ladykiller perk (as an example)
