Been fighting with this problem for a good while now. Looks Menu seems to work fine itself, and certain of my presets are also working just fine. However, some (and of course they're my favorite primary player characters) are absolutely broken and look like this as soon as they are selected.
I'm running a pretty heavily modded game, with many body tweaks and edits, but those are mostly female focused. This bug is affecting both male and some female presets (for females, specificlly the bottom two Agemo NPC replacer faces that were added as presets). I've tried turning mods on and off, reinstalling Looks Menu, tweeking the load order, but no dice. After a great deal of google digging I found one other person who was also suffering from this issue but he never got an answer.

When you have Buffout 4 installed all the thing you need to do is locate file Buffout4.TOML
Open it with Notepad++ and find it down below the line [Compatibility].
There you need to change FALSE for TRUE just like in the picture) SAVE the changes and you are good to go)

Thank you so much! Now for my next and most game breaking bug - Figuring out when I CTD on every load of every save... That was the bug that started my total profile rebuild, and it's come back now that I've got Sim Settlements 2 and some of it's add ons installed... Something I've got to fix because I'm so excited to play that mod.

Also randomly pops up on a working load order. In that case I can usually clear it by clicking the preset a couple times, or doing as you suggested and clicking a different preset and then going back to it. Other times I need to reload the game and it randomly decides to behave. Long story short the presets just tend to be kinda temperamental in my experience. Current load order has EVERYTHING working EXCEPT for my favorite female face. I use Nora Alive mod, and my perfect wifu preset for some reason has the jaw and nose of the wicked witch in Wizard of Oz :-(
May not be everyone's issue, just commenting in case others have the issue as a place to start.
Feature Creep is what occurs when new features are piled onto an already completed file, and usually breaks other features already implemented within the system.
Mod authors should reframe from constantly updating their mods in a way that changes or adds new features ontop of an already finished mod.
When that happens, new game breaking bugs are bound to arise. Once their mods are to a functioning state, it should be left alone, unless any bugs occur that need to be patched. imho Looksmenu didnt need anything else added to it, the only fiddling was to be updated to be compatible with undated fallout 4 exe (and even that's rare) so that it can be used with newer fallout 4s. But that is also redundant anyways, because there is already a mod that can downgrade the fallout 4 versions to their previous versions before. That means mods really dont need to be constantly updated like usual unless it's what the author prefers, users can just go back to their original files easily.
Looksmenu was compete. Leave it be. Now that this brand new bug has been introduced, all hands on deck must scramble to try and fix it, and it's extremely hard to know where to begin.
Very stressful stuff when things break like that!
See Stavetskaya's reply that I stickied here - Still doing tests BUT so far this has been the absolute cure for me! Now to see if I can get ENBoost/ENB back without the problem returning.
Sometimes I get the blown out face mesh but if I click the face again a time or two it'll load properly. As long as my in game NPC's don't have this issue (which so far none have) than this really doesn't matter. I've gotten to the point where this bug is gone or fixable by clicking it again, but still juggling the load order because now some or all my presets are getting dark face bug.. I swear as soon as I fix one thing I break another =_=
EDIT: For anyone following all this and having similar issues, I also figured out why some of my Presets like my Agemo's NPC Replacers presets all had brown face bug - It was the LooksMenu Customization Compendium optional download 'HD DLC Fix'
Ironic that something I downloaded specifically to help with this issue would end up causing it smdh
This preset issue was going on before testing Buffout to fix that, so Buffout is a fresh install and has all it's prereqs. LooksMenu is updated to 1.6.20, and if memory serves it was after downloading that most recent update that I starting having problems..
EDIT: While removing ENBoost and all my enb files from the game folder eliminated the constant un-fixable broken head texture/mesh, I can still get it depending on load order.. So weird considering I really don't have any big alterations to male characters... I'm thinking this is primarily a bug with LooksMenu and/or the LMCCompendium as they control the presets and affect all of them.
IDK if you noticed my reply to radbeatle below, but Buffout definitely was not causing my texture/blown out mesh issue. As bad as the LooksMenu problem was, my truly game breaking problem was that all my saves were CTD (due to some recent big additions to my mod list AND upgrading to a new PC, I was in the process of rebuilding my primary profile). It was suggested to me that Buffout might be a fix for the crashes, so it's freshly installed as of the past few weeks, while my character presets have been broken for months before even moving to my new PC. Buffout didn't fix or cause either issue..
All this being said, I just figured out what IS causing my preset head blowout problem - IT WAS FREAKING ENB!!! I wasn't running a preset, I only wanted it for ENB Boost (felt like I needed it on my potato build; probably don't for this PC but wanted to have it installed so that I could eventually look for a cool preset to add later). Last night I discovered it was enbseries config files being active that was causing a nasty blur/dirt/damage overlay that would pop onto my screen when I would look certain directions. Turned that off and issue was fixed. Today I gave up bug hunting my CTD issue and disabled all my mods and started reactivating a few at a time. My second batch was my character making files, upgrades, and presets - Instantly ran into the blown up head bug. Figured while I was here getting this working I'd fight with it and nothing at all would work. No idea how or why ENB would be connected but decided what the hell and deleted all the enb files in my FO4 folder and BOOM, my heads are no longer spontaneously combusting! So cool to find the culprit, but kinda sucks since I did have plans to finally get into the fancy ENB world of gaming..
