So I finally realized what people are having issues with regarding this mod. It seems obvious but I'm so used to FNV/FO3's GECK storing scripts in the esp that I never uploaded the script with the file. 1.4 includes the script now.
Version 1.3
This version is more or less 2.0 without any new features, as I've simply reviewed my entire mod for errors and merged everything into one version. 1.3 removes the effects of the mod from the Black-rim Glasses and adds a new duplicate item called Prescription Sunglasses that have the mod effects on them. 1.3 adds the immersive version as a core feature, so now Eye Glasses and Prescription Sunglasses will penalize anyone who wears them that does not have the perk from this mod (See below for more details).
Immersive Optional Version (core feature as of version 1.3)
Added an optional version of this mod that basically causes the entire reverse of this mod for those wearing Eye Glasses and Black-Rim Glasses without the mod's perk (-2 PER and blurred vision). In other words, prescription glasses are now only intended for people that actually need them (or if you're feeling silly you can run around with them on for fun); PA helms are an exception to this and will not cause the reverse effect.
Version 1.2
Added a PrescriptionHUD effect (+1 PER) to all the PA helmets that will only activate if the player has the perk from this mod; this will keep the power armor helms vanilla for characters that don't use the perk from this mod but still provide functionality for PA helms for characters that do.

Description
Mod really isn't a conversion of Superonofa's mod but it was entirely inspired by their work.
Mod is pretty straightforward. Outside the Vault 111 elevator platform in the trailer with a switchblade stabbed into the terminal is a magazine tucked into the box of files on the desk. Read the magazine and you'll gain the perk "Visually Impaired". After that your vision will be blurred while not wearing Prescription Glasses or Eye Glasses.
Both glasses are craftable at the Chemlab under the "Glasses" section.
If you want to remove the perk just type Player.RemovePerk xx000f9a

Installation
This mod is just plugin so no need to worry about overwriting any files. Add it to your data folder and active the esp in whatever way you normally would.
Compatibility
This mod does not work with any mod that alters the Eye Glasses or forces you to unequip them (mod is compatible with power armor helms). If another mod is incompatible, let me know and I'll see about making a compatibility patch.
In the images you should notice some active/inactive titles. I've tested this mod out using Jet, night vision scopes, VATS and the Bullet Time mod; the blurred vision IMOD (visual effect) works fine with them all so it should work with everything else. If anyone notices a bug with any IMODs let me know so I can look into it.
