The IV Stimpak works exactly like a normal Stimpak but does not dehydrate you. It is lore-friendly, requiring sensible materials to craft. Take one Stimpak, one Purified Water, one Antiseptic, and three Plastic to mix an IV Stimpak. This does not require any perks and can be crafted at a chem workstation under health (the same place you craft normal Stimpaks).
Note that this is a different item than a stimpak and cannot be used on companions, so you might want to carry a few of the originals for your friends!
The mod works in all difficulty modes, but only makes sense in survival.
