In order to obtain the mask you'll have to add it via console "player.additem 17000F9D" or use a in-game explorer of your choice.
As for the outfit, it replaces the Letterman's jacket and jeans which you can either find in the wastes or by using console.
The mask has been ported from Payday 2, I've modified the textures to give it that nice worn & lore friendly look.
I recommend using MO2 for the installation.
