Perhaps a staff member who was supposed to repair House's reactor? But there was an accident and they hit their head, leaving them unconscious near the reactor and thus they became one of the first Ghouls.
The radiation sustained them as they lay unconscious for days, during which the bombs fell and they were trapped inside.
Only to awaken with no way out and silence from outside...
All this time.
Mr House kept them to study Ghoulification and came to see them as a pet.
