The armor is a total mashup incorporating the cyber arm, as you mentioned, grafted onto a sorta-retextured version of Ulysses' duster with some Combat armor and other bits and pieces. Add in various tweaked accessories from Nivea's mod that I mentioned elsewhere here and the flipped around eyepiece thing. Finally, cap it with a reinforced Desperado Cowboy hat.
It steadily accrues crap as I go along, like I imagine it would in the Mojave. Combined with the Signature Armor mod, it feels pretty damned customized and personalized. I like it.

As with my reply to KamSolastor regarding the backpack, the bandana is from Nivea's Spice of Life mod, and it has similarly been modified slightly by myself. Nivea's original mesh doesn't hang quite so low past the chin.

Yeah, I'm fairly certain that's where it came from.
I'm not sure how frequently the original object crops up in-game, in leveled lists or whatever, or where it may be sold by merchants, because I extracted the mesh from the .bsa so that I could make my edits and then add it to my personal armor compilation.
