As said when you start to worked for some time this kind of issues shows up when GECK LOD generation. You can have set up all good, generated lot of times, and only changed things on the main land in center never touching the rest of the world and still at a certain point start to get them. No matter what you try to smooth or flatten these "holes" in GECK will not change the output in general.
I might have a solution but need to test it a lot more to actually give proper advice how to fix it.
