Makes the "good" Super Mutants evil, with the exception of Mean Sonofabitch, so that you gain karma for killing them.

All Super Mutants in the game now have evil karma, with the exception of Mean Sonofabitch.

Why?

Karma in Fallout New Vegas doesn't reflect whether a character is good or evil at his or her core - I don't know whether that's the system the developers intended to have but it isn't the one they implemented in the game - rather, it's a collection of all your past actions. It's also absolute in the way it judges morality, so for instance, stealing is always bad, even if you're stealing to survive or from an extremely evil group such as the legion.

Sometimes, karma is assigned to a creature that may not be evil per se - in fact, may not even be able to be evil, such as the radscorpions - but are a threat to the population and, as such, killing them is considered a good deed.

Super mutants are monsters who committed horrendous atrocities in the name of their fanatical leader and the supremacy of their group. They're also infected, diseased creatures that suffer from all sorts of problems due to their condition. Yet in New Vegas, as long as they don't actively want to kill you, they're considered "good", and killing them, even as mercy to free them from their suffering, brands you as "evil". And that doesn't sit well with me.

"But what about the good mutants, Marcus, Lily, Neal, Mean Sonofabitch and Dog/God?" I hear you say.

- Marcus doesn't have much to say in FNV but talking to him in FO2 reveals a lot about his personality. He has no remorse or regrets about anything he did in service to the Master. In fact, he speaks in glowing terms about the lunatic that tried to enslave humanity by turning everyone into brain damaged abominations. He still believes in the Master's project and even admits he would have no qualms about dipping his former friend into the vats against his will. Marcus has simply accepted the fact that the Master lost and his project is dead. It's even hinted multiple times that he isn't even peaceful out of genuine conviction, he's simply trying to make peace with those he considers inferiors because he doesn't want the remaining mutants to be wiped out. 

- Lily seems more brain damaged that evil to me. I suspect that the character was supposed to be senile before she was mutated, and the FEV only worsened her condition. Regardless, like I said before, the game is absolute when it comes to morality and she has a ton of crimes against humanity on her back. Considering all her mental issues, killing her seems like euthanasia to me. 

- Neal, the rest of Jacobstown Super Mutants and Dog/God have evil karma mainly because of their past actions.

- Mean Sonofabitch keeps his good karma because he's actually atoning for what he did by helping the people of Westside and killing raiders. And also because unlike any other mutant in the game, killing him would actually affect people negatively.
