Wanted to add some extra spice to an enemy with a special "Bleed" effect they can inflict. I didn't want to settle for a basic DOT, so I added some seasoning to make it more distinct.

The bleed damage is low-ish but lasts for a while, totaling to a  significant chunk of health. (Lethal for low level characters.)

Bleed can't be instantaneously cured like Poison, but you can avoid  being inflicted with it entirely by wearing Heavy Armor.

These ghouls only inflict the bleed effect with their left-handed attacks, but they favor their right hand attacks. I like how this feels from a player's perspective as it makes it slightly uncommon, and you *know* if you're going to get inflicted or not once you make the Lefty connection.

When bled, a gruesome "Slash" sound effect plays, a text warning appears, your character let's out a pained scream (from the "Death" topic.) and a red tint covers the scene, which fades away once the bleeding is over. Purely cosmetic effects, but it adds an impact to combat the vanilla game (and mods) noticeably lack.

To be an overachiever, I also learned to add a Pause Menu Help entry (via script) to explain the bleed effect and the Heavy Armor immunization in-game.

(These ghouls do a lot more damage, I just modded my health to a high value for demonstration purposes.)
