So for my custom mod I have come pretty far already...
This is an update on how it migh look...
What I have added/changed so far:
Skintexture: Added grain to it and texture so it looks more realistic
Skincolour: Added colourvariation on several parts of the face and body
Lips: Changed both texture and colour
Tattoos/scars: Added a few options that give some tweaking possebileties
Glossmap: Added separated mesh to add different gloss maps but also customed a new alpha

If I want to get a max gloss effect I use a normalmap with an high gloss factor and in nifskope I use the material EnvMap2 and set the Glossness and the Specularity to maximum ..
It's a combination of normal maps, alpha layers and gloss factors.
