This patch is correcting problems that occur using both Topal Island and Better Cities. The latter is placing the Spearhead Tower at the same spot as Topal Island.
You must have this load order: 1) the Better Cities files, 2) Topal Island and 3) the patch.
You now can use both mods without any bigger landscape or transport problems. The patch has a new boat to the Spearhead Tower.
