   It's indeed the Berserker Armor and the bandana (and gloves) are part of the Iorveth armor mod.
   
   I made an .esp file for myself, with changed stats and realistic recipes, now calling it Mercenary Armor, with the Knights greatsword from Jaysus swords mod and an Imperial bow with Steel arrows in an iron arrows quiver (got that from the private singular quivers mod), now named Mercenary Sword, Mercenary Bow and Mercenary Arrows

   Looks like the armor itself is the Berserk armor, the Hat is from the Witcher armors
