Fulfil all the requirements.
Check that you have all the listed mods in the mod's description page (sometimes in the sticky) downloaded, installed AND activated before you attempt to load a preset. Also make sure you activated the preset. Use "Preview file contents" to check the name of the preset and the sculpt in the mod's download page.
Select the Race and Sex.
Optional Step: High Poly Head
Under the Head page in the Sliders tab, go to Face Part and change to High Poly Head.
It's usually the 3rd or 4th option.
Go to the Preset tab, load the Preset with F9.
(If it looks weird, continue to step 3, otherwise you're done)
If the head looks weird / different from the screenshots, it's because you don't have the same mod sliders that the creator used. Go back to the mod's page, read through the description and check the sticky to make sure you have the same mods installed*. If you've tried the above and it still doesn't look right, proceed to the last step.
Go to the Sculpt tab and load the sculpt with F9.
Done.
Extra reading:
- Not all presets look good with all races. Check the mod's page to find out the race the author used for the preset. This entire process is easily repeatable so you can try it with other races.
- I've seen preset pages with no facegen files (the NIF and DDS) because some creators didn't know they should export them, and also some rude af one-liner comments demanding these files. I've always managed to correctly load downloaded presets without the facegen files by following the requirements but to be fair there are some authors who forget to list the overlay and slider mods they used correctly as well. Either way, loading the facegen files will solve all issues, so if you can't get the same look even after following the instructions above, stick a picture (spoiler-tag it for courtesy) in the comments of that page and wait for the author / others to respond. Don't expect too much help since the preset will load perfectly on the author's set up and he / she can only help by double-checking the listed requirements.
- As far as I know, the makeup applied to a racemenu preset is "locked-in" to the preset and the DDS file. I have never attempted to change the existing makeup of a downloaded preset, but I assume that if you load the sculpt with F9, the makeup loads with it (I could be wrong here). Presets (much like followers) are offered by authors as-is, it is not the uploader's responsibility to help you with further editing what is provided. Once you get to Step 3, the head should look exactly as it is on the mod images and that's that. If you've downloaded and installed all the requirements (and the author has also listed all of them correctly), loading the preset without loading the sculpt should give you the same head as the images, and it should be easier to remove the makeup from there.
* This is why I always tell people who download my preset files to download Expressive Facegen Morphs on literally every single mod I've uploaded that contains a preset.

And yes, I did try Custom Races and mods like that, but either there's an issue with the skin (neck seam or dark face), or the custom race doesn't have a "textures" output.
suggest you use the 1.597 game version. since it supports the racemenu mod, found here in
nexusmods. Correct me if I am wrong, I think the author
of racemenu have an updated version in his/her patreon that supports
the latest version of the game so it's better to check it out as well.
Also, once I hit done, do a save and reload, I get this weird bug where my face elongates, happens with any race and sex, very strange.
Expressive Facegen Morphs for instance is a completely hard requirement in the presets I shared here on the nexus, otherwise the face will look super different.
Of course, simply importing the edited sculpt is also good, but again, without this mod the same preset will look really bad actually.
Also useful for your vanilla NPC, it makes facial features smoother and not so... squarish?
