This is the Voice of Namira. Found her in Reachcliff Cave in the boss room. That is during the quest to get Namiras ring, the quest "The Taste of Death".
Wiki: "Though Namira makes no visible appearance, she's voiced by an unseen NPC named Voice of Namira located in an unreachable room east of Namira's shrine."
 
She should not be there....
