As for the mod, it's been a few years and I've since changed my list many times over, but I'm almost certain its the optional face mesh of this mod. The date I downloaded it and the date I posted these screenshots line up, so it's a pretty safe bet!
Though I did have the feeling you'd enjoy this character especially
Her design was definitely inspired by my kitties, so it makes me happy that she can remind others of theirs as well! ^_^
