Long story short, I got annoyed that one of the players two favorite companions was guaranteed to die during the main quest.

For v1.1+:
The menu has been added which should trigger when the Short Sighted quest starts.
A second chance is given after helping out on the eye if the quest has already started, though this is experimental at best and may cause softlocks!

For v1.0:
To fix this issue quick and dirty, the companions are hard locked to be:
1) Sam Coe in the Eye
2) Barrett in the Lodge
As I didn't use them during my playtrough. If you want to substitute your own companions, I suggest using a newer version :).



The companion dies during MQ204 (High Price to Pay) but is selected during the start of MQ201b (Short Sighted) at this point the companion going to the lodge will be locked in as a follower during MQ202 (No Sudden Moves).

There also seems to be an option to skip MQ204 in new game plus by telling Vladimir to move the artifacts after the initial constellation meeting.
