This is an Open Source and totally free Controller mapper. You can download it from github.com. See the link below. Pick always the newest Release.

Run the portable program with administrator rights or the controller will not work in-game. In the Download section you can get the profile specific for the game.
Configuration:
Left Joystick = 'W,A,S,D' (Character Movement)
Right Joystick = 'Mouse' (Camera Control)
D-Pad UP = 'TAB' (Non-Combat Mode)
D-Pad Left = 'Q' (Steel Sword + Switch sword stances by pressing the same button multiple times)
D-Pad Down = '-' (Next Magic Sign)
D-Pad Right = 'E' (Silver Sword + Switch sword stances)
Back Button = 'J' (Journal)
Start Button = 'ESC' (Cancel/ESC Menu)
Guide Button = 'F12' (Screenshot)
X Button = 'CTRL' (Allows you to split item stacks while in your inventory.)
Y Button = 'I' (Inventory)
B Button = 'SPACE' (Pause)
A Button = 'Left Mouse Button' (Interaction)
R Shoulder = 'M' (Map)
L Shoulder = '6' (Quickslot)
L Trigger = 'SHIFT' (Cursor Mode, Move the Mouse in-game)
R Trigger = 'Right Mouse Button' (Interact with inventory items/Use Signs in Combat.)
