The eyes are from "A Large Set of Eyes for Geralt" mod found here: https://www.nexusmods.com/witcher3/mods/3754 I believe it is eyes number 1 or 14.
But I also use the normal maps from Ramccoids peepers mod. dl the normal maps version only in optional section, and give priority over the other eye mod: https://www.nexusmods.com/witcher3/mods/1974?tab=files The hair color is the milky white hair and beard color only option from the YAHW mod by nexuer. This is an old screenshot. I usually use Banjomir777s cgi geralt remasterd mod for face textures. Highly recommend. This obviously can't be used with the geralt redone face though. You can find Banjomir777s retexture here:

He also has a great head mesh, that is probably the best one iv used but it has beard issues(that he is aware of) if you want to use non hair-work mods. Still highly recommended.
Otherwise I use reeves killing monsters v6 mesh as an old solid staple, which goes great with banjomirs face textures and BzDanilkas grey eyebrows for 3d eyebrows and yourpietys hazel eyes.
