As someone who just had an emergency orchiopexy to fix this very issue, I feel well within my rights to twist some twits up. Also, props for the custom art on the little spell window.
My 'run in' with this was 28 years ago, and I came here thinking 'This shouldn't be funny. But it is.' ^_^ I also find myself wishing it was 'sane' efforts to implement it as a round-by-round damage increasing effect.. with an extremely increasing chance of prone / taking no actions due to pain check. Some of my friend's clearest teenage memories are simply titled 'Six hours of agony and nearly punching a doctor who asked "Does this hurt?" before poking something'. :)
Also, congrats on managing to get the fix! Hope your recovery went nice and smooth.
It should appear with level 2 spells, when player is level 3. Perhaps check if you are using script extender and try to respec if it still does not appear when you are level 3.
