It's not mentioned in the mod description but for people on the Epic version who find it doesn't work : Playing Neighborhood Watch disables the mod and it must be reactivated, any time you play NW you have to run the mod again for it to work in single player - even if offline.
Edit: You need to reenable it every time you start the game, it seems.
Trojan.MSIL.Killmbr found, many sites including NordVPN said the .exe was good but I still detected this trojan on two other sites. I would recommend not to run it.
