Doesn't seem to make a difference for me. Running an expensive gaming tower (granted, with a few mods but nothing that is extensive in adding content). Shame Bethesda are cheap bastards now and refuse to make a decent engine, instead of recycling the same one as before. Same crap, same problems, same piss-poor performance.

I'm now running a very smooth game compared to before. Still a little stuttery in urban areas but that is down to the engine not being able to handle it. Nothing to do with my tower or it's performance.

It's a small price to pay for almost full-Ultra settings and running around with roughly 200 mods. I posted my settings, graphics cards settings and .ini tweaks here. Scroll down a little and you'll see a big post by me.

Feel free to take a look and see if those changes help.

This is counter intuitive. Borderless full screen ALWAYS runs WORSE than exclusive full screen on Windows.
It makes no sense to go Borderless performance wise. You use borderless to avoid alt+tab crashes.

Be grateful you can use fullscreen, if you buy a gaming laptop and it dares to have optimus technology, Fallout 4 will refuse to play at all in exclusive full screen.

Sorry a little late but try to change iPresentInterval=1 to iPresentInterval=0 in both Fallout4.ini and FalloutPrefs.ini. It does same s*** to me i thought Fallout4.ini would work but it needs both for it to work. went from 48fps to 117-143(capped)fps. Make sure to set read only on FalloutPrefs.ini so it doesnt try to change it back if you adjust graphic settings again. If you want to change graphic settings, you have to tick off the read only then do it but put it back on after changes. I had to reinstall i thought something was wrong with files because i wasnt using mods so i just mess with .ini for few hours and finally figured it out.

I have an i7-3770 and gtx 1050ti, running Windows 7. 1920x1200, medium-ish settings. Vsync on. Outside of a very few downtown or other bad spots, I had a generally silky smooth 60 fps performance. That is, however, until I got into big fights, or spun around fast or run a lot, all of which would cause some micro-stuttering, especially if I was having to follow someone running ahead and to the side of me on a quest.

What mostly fixed it for me was, in Nvidia Inspector, in its dedicated settings entry for Fallout 4, setting the framerate limiter to 58 (v2). This is on a monitor that natively runs at 60fps. If you don't have/use Nvidia Inspector I think you can also set it in the prefs ini, using iFPSClamp=58 in the general section, but I haven't tried that, and doing it that way can cause problems (see this iFPSClamp=58 steam community thread).

I leave the game's own default Vsync enabled and made no other changes (and use full screen mode, not borderless wondowed) but I do use a couple of possibly relevant mods: No Godrays Whilst Keeping Volumteric Lights and Texture Optimization Project.

14/01/21 edit: Almost forgot, though my mix of settings weren't maxing out with TAA enabled (gpu 85%, cpu 45%), I found dropping to FXAA antialiasing also helped eliminate residual microstutter in high-action moments of running or spinning around in fights etc.

the issue for me is that doing this doesnt give me a full screen of fallout, like is supposed to happen when opting for the 'windowed fullscreen' option, there is always some part of my desktop background showing, i really would rather play in windowed fullscreen but oh well

I would have never have though to try not using the full screen ini. And to my surprise it worked, I noticed a difference right away.
