Known Visual Bug
When you get Hit by an Enemy the small PA Model on the left shows that you lost a Piece from your Armor but this is just a Visual Bug. The PA is still Intact.
If you have any Mods installed that touches the Health System of the Power Armor in the Base Game and the DLCs, make sure this Mod is loaded after them.
