Edit: I got it to work by following @SCP682 and @medhatobo's instructions. After replacing the X in chunk0patchX.rpkg, I added the new file name to the mod manager to activate it. (E.g. I added chunk0patch9194.rpkg to the mod manager, renamed the mod name (In my case, I renamed the mod name to "GigaChad 47") and activated it.)
works for me, just make sure you add the rpk or whatever the patch file is called to SMF, and dont forget to go to the site mentioned at the top comment here that has instructions, ill paste it below

I haven't tested it too thoroughly yet, but in my experience it doesn't work with a lot of disguises. I haven't had the mod for too long and from the screenshots it looks like its supposed to work with some but from what I can tell a lot of the disguises are broken.
