That's easy, space aliens abducted him as he was laying there dying, they replaced his body with a dead one so no one would be the wiser. He eventually escaped, and made his way back to Earth, but those experiments they did on him left him unable to age, similar to the young girl in the F03 DLC Mothership Zeta.
The best solution I can think of is to make Randall into a ghoul instead. At least that would explain why he is still alive after two hundred years. Only problem with this scenario is that I haven't found any irradiated locations in Zion Canyon unless he was ghoulified after leaving Zion.
He could have been irradiated from the bombs that hit Salt Lake City before he went to Zion, drinking from a pool of irradiated water only put him over the edge
As a way of explaining how he's still alive during new vegas (if you ever decide to add dialog or even a quest to the mod) you could probably say that his body had an abnormal reaction to the plant/fungus that he got infected by from his wife from vault 22. Instead of killing him and eventually turning him into a spore carrier, the spores mutated and formed a symbiotic relationship with him that has been keeping him alive the past 200 years (kinda like Harold from fallout 1-3). He looks perfectly human on the outside, but every inch of his innards are infused with plant and fungus material that keeps all his organs and brain perfectly intact as long as he eats and drinks enough to keep the fungus sustained. You could even integrate that into the mod, like give him a perk that makes him fight better in sunlight, or make his blood green from all the chlorophyll in it. As for the skeleton you could have him explain that he went up there thinking he was gonna die, but then the strange plant mutation in his body kicked in and "revived" him, just as he was being discovered by a sorrows tribal, and he suddenly woke up and accidentally killed them thinking it was any enemy. Killing one of the people he had spent so much of his life protecting and raising turned out to be the last straw, so he set the corpse up to look like it was him, and then fled Zion to try to escape all the hardships he suffered there.
I would say that this works in a weird, Fallout kind of way. He mentions coughing with blood and shortness of breath which he believes to be lung cancer. You could have so it turns out to be this fungal infection, and you find out by taking him to that doctor whats-her-face that you talk to when you do the Vault 22 quest, who asks you to find her ghoul doctor friend. Or you have talk to the ghoul doctor lady. I will say though, it does seem a little lore unfriendly however you do it though, and there was an almost poetic beauty to his story's ending, but this still looks great.
EDIT: Make sure he can get the Survivalist's Rifle and his armour, otherwise it'd be like Boone without his rifle or beret.
I'm glad I'm not the only one that found Randall Clark to be one of the most interesting characters I've ever read about. Downloading this and putting it in my game. Getting it voiced would be awesome but that might not be in your power as of now.
