I don't think I understand what you mean. Is there another fast travel menu other than clicking the marker on the map? If not, then this mod does exactly what you want :)
If you want a mod that makes them greyed out but still visible and only after you traveled to them it makes them available for fast travel, you can use mod like this: https://www.nexusmods.com/oblivionremastered/mods/259
I've also noticed this on doors leading into the cities, which is weird because I didn't even touch those. Do you remember where else you've seen the [NL] tags? I'll try to sort this out, but more information would be appreciated.
Hmm, maybe it's because I didn't set Altar ESPs as Parent Masters? Also, is that screenshot from xEdit?

Wish this was out sooner, now 7 hours in my game and I basically made a rule for myself that I cannot fast travel to a city unless I at least walk up to the city gates myself.
Hmmm, I guess I must've misremembered. After all, it's been a while since I last played Oblivion, must've confused it with Skyrim's fast-travel mechanics I've updated the mod's description.
If you install the mod mid-playthrough, you need to rediscover the affected markers again. That means stables around and gates to every city. They won't be hidden permanently.
Oh, looks like somebody beat me to it, wasn't there when I checked yesterday It seems that Samantha's mod works even better, since according to the description, it can be installed mid-playthrough!

The only city that wasn't given a marker from the beginning was Kvatch.
