For some reason, the mod just dont work. I installed it but, i didn't chaged anything... Not even the Wig in her inventory, it is like i never installed it.
Make sure the mod is at the very very very bottom of your plugin load order and MO2/Vortex load order. If you see no changes at all, it means another mod also editing Lydia overwrote mine entirely
I noticed the head nif, 000a2c8e is LE Skyrim format (NiTriShapes). Might be why some people are having trouble reading it. I updated the head to have BSDynamicTriShapes using Nif Optimizer and it seems to work now.
I've got this minor issue with the mod. Every time I boot up the game, it loads up High Poly NPC's model of Lydia even though I've set Ultimate Lydia to override it in case of conflicts. It makes it so that I have to disable and then re-enable the mod in my mod manager each time I boot up Skyrim.
This, for me, IS the Lydia change MOD to use. She's a really stunning girl and easy to set up with my CBBE curvy set up. There was that glitch with the missing ring around her neck. But following the instructions was a piece of cake and now she's in one, and very desirable* piece. Thank you 8-))...

1. Your load order: if another mod editing Lydia has its ESP below mine, then mine will be overwritten and you might get the gray-face. 2. Your loose files: if another mod editing Lydia has its facegen loose files overwriting mine, then you might get the gray-face.
If you're not sure, just try putting this mod's ESP plugin at the very bottom of your load order and overwrite all loose files with mine.
